Chin Advancement (Advancement Genioplasty)

Overview of the surgery:
Chin Advancement (Advancement Genioplasty) is one of the most advanced, precise and sophisticated types of chin surgery to take the lower front part of your chin and bring it forward.  This procedure is more involved than placing a chin implant and requires making incisions in the bone and also stabilizing it at the end with space age technology of rigid fixation.  In some cases bone grafting in front and the top of the chin plate may be necessary to create an attractive final results

Duration of the operation:
The procedure will take approximately 30 minutes to perform

Post operative care:
Antibiotics are prescribed to control infection and swelling.  Cold compress will be applied to face for 24 hours. Your chin will be taped following surgery. Do not remove the tape and reinforce it with additional tape if needed. Take your medications on time and finish the course of treatment. For the first day, liquid food is advised, cold food for 24 hours then soft food for the next five days. Rinse your mouth three or four times a day with warm salted water may brush your teeth if the brush is soft and you are quite careful to keep the bristles away from the stitches.  

Recovery period:
The recovery takes one to two weeks. Most people are back to work within a week or two. We encourage our patients to return to work as soon as they can within few days after surgery.

Possible risk and complications:
As with any surgery, there is always a possibility of complications following chin surgery, including infection, bleeding, a reaction to the anesthesia, hematoma (localized swelling filled with blood resulting from a break in a blood vessel), seroma (a mass or swelling), extrusion (the implant works its way back up to the skin surface), capsular contracture (excess tightening of the scar tissue) which may distort the implant, asymmetry, nerve damage and bone erosion.

Anesthesia use:
Local Anesthetic (while you are awake), combined with a sedative to make you drowsy. You'll be awake but relaxed.

Hospital admission:
This procedure may be performed in the cosmetic surgeon’s office-based facility, an outpatient surgery center, or at a hospital. This procedure is usually done on an outpatient basis.

Preparation before Surgery
  • Avoid aspirin, any aspirin containing medication or any other non-steroidal anti-inflammatory drugs (NSAID), such as Motrin or Advil, for one week prior to treatment. Because aspirin thins the blood, it can interfere with normal blood clotting and increase the risk of bleeding and bruising
  • Smoking inhibits the healing process, so stop smoking before your procedure and if you start again, make sure it is after you are completely healed
  • Avoid drinking alcohol a few days before your surgery
  • Make sure to not to eat or drink for at least 8 hours prior to your surgery
  • Make sure that you arrange for someone to bring you home
  • Assessment of the medical history (any allergies, serious medical condition and all medications taken both prescribed and non-prescribed), physical examination, and laboratory tests will be performed during consultation.
  • Blood and urine samples will be collected for routine preoperative laboratory tests. X-rays may be taken to evaluate the individual's bone structure.
  • Smoking must be avoided for about 3-4 weeks prior to surgery, as nicotine interferes with circulation and will greatly affect healing process.
  • You will likely to be asked to stop drinking alcohol, a week before the surgery and throughout your recovery period.
  • Avoid taking any medications such as hormones, anticoagulants, anabolic steroids and supplements at least 4-6 weeks to prevent complicating medical factors prior to surgery. Avoid taking aspirin, anti-inflammatory drugs and herbal supplements as they can increase bleeding.

Augmented Rhinoplasty (Nose Augmentation)


Augmented Rhinoplasty is very common among Asians and non-Caucasians to correct a flat nose with a round bulbous tip and flaring nostrils. The aim is to build an aesthetically pleasing nasal bridge with an elegant projection and well defined nasal tip, and at the same time narrow the flaring nostril base. An implant is usually used.

The implanted material is biocompatible and extremely safe. Incisions are made inside the rim of the nostrils. Soft tissues of the nose are then separated from the underlying structure to create the appropriate space for nose implant projecting the nasal tip and increase the height of the nose.

Of all the procedures performed by plastic surgeons, rhinoplasty is the most challenging. And it is with this procedure that society judges you by its shape and the skills of the surgeon because the nose is the first thing you see when you look at someone. There is no way to hide the nose, before or after the procedure. Noses are so variable in size and shape that it is hard to define what is normal. Every nose is different and every result should be different, fitting the patient’s face.

Nothing defines your appearance quite like your nose. Like it or not, people identify and even judge you by its shape and size. If your nose overwhelms your other features, it can hamper your self confidence by affecting your looks. That’s especially true if you are transitioning from your birth gender as a male to your new gender as a female. Having the nose that is bigger and wider than the small, narrowed version found in most women can ruin the feminine identity you wish to project. Can it be changed? Yes. By reshaping your nasal contours, rhinoplasty is one of the most important operations in facial feminization.

If you’re a transgender individual, your nose presents one of the most obvious facial challenges to your feminity. Distinct differences are found between the sexes. First, male nose are larger in every aspect than in female noses. They’re longer and wider with larger nostrils and generally bulkier tips than those features in women. Second, a man’s nose points either ahead or slightly downward at the base, as opposed to a woman’s nose which points slightly upward at its base. The female bridge in some ethnicities also “scoops” or dips before sloping gently straight downward or into a slightly upturned tip. 

Third, and perhaps most important, the angles between the nose and the forehead, and between the nose and the upper lip tend to be sharper in men than woman. As a male you may not have the chiseled profile but your nose likely projects off your forehead in a more abrupt way. You may even sport an unsightly bump, called a dorsal hump.

In changing our nose to fit your new gender, your surgeon will make sure that it not only slopes gently from your forehead but also is free of any humps or bumps. He or she will also downsize and resculpt until this part of your face fully reflects your new identity.

Temporal Fossa Augmentation


The temporal fossa is located above your cheekbones, just to the side of your eyes. It is the shallow depression of your skull.

You know it as your temple. Created by an underlying pair of cranial or temporal bones, it houses the temporalis muscle, one of the strong muscles necessary for chewing. If you take away the muscle, you are left with a space called the temporal fossa. Normally, this indentation is a discreet hollow just below and behind your forehead. But if the depression is so pronounced that it makes you look sickly or gaunt, you may want your surgeon to fill it in. Augmenting a temporal fossa depression isn’t a necessary step in feminizing your face, but it will add immensely to a healthy, feminized look. 

There is no difference between the sexes when it comes to an exaggerated temporal fossa depression. Even though I have treated only one woman with a significant indentation, I believe strongly that a deeply concave or inwardly curved space has nothing to do with gender. Whether you are a man or a woman, a pronounced hollow just behind your lateral orbital rims, the bony ridges to the side of your eyes, can have a significant effect on your facial aesthetics. This hollow can make you appear starving when you are well fed. Although the majority of the people don’t have a temporal fossa depression that merits attention, it is not an uncommon occurrence.

The degree and precise location, however, can be variable. The depression also isn’t necessarily symmetrical or the same on both sides. When it does exist, augmenting the space can offer many aesthetic benefits. Improving the contour can create a more appealing look.

Mandibular Angle Reduction (Reduction Angleplasty)


Mandibular Angle Reduction is usually done to individuals who have wide jaw line or square face. It reduces the width of the lower face and change a square face to an oval one.

This procedure can enhance the masculine features, in men while in women it can give an impression of strength and confidence. In some, however, the jaw can be disproportionately wide as compared to the rest of the face or zygomas and mandibular angles are overly prominent that upsets the balance of the face.

This is one reason for aesthetic Mandible Angle Reduction. The incision is commonly done inside the mouth which means the scar will be invisible.

Lower Jaw Tapering- Angle Reduction/ V-line mandibular Angle Contouring

Tapering the mandible or lower jaw is an important step in facial feminization surgery. Women have more tapered and narrower lower face. Men have considerably prominent jaw line rendering a wide, square face which enhances the masculine features. With these differences, the surgery is essential in transitioning from male to female. Thus, improving femininity and eliminating obvious masculinity.


It allows for reduction of the chin to a more slender and oval shape without increasing its length. The surgery also reduces the overly strong bone flanking each side of the chin, thus transforming the more masculine U shape to a sharper and more feminine V shape. The chin line is excised into a smooth line. The chin end can be brought together without touch of the nerve. The protruding parts in both sides of the mandible are excised. Chin Reduction


The chin is one of the most important features in gender identification. It is so different between the genders. In men, chins are vertically higher and wider than in women. The chin is the front portion of the mandible or lower jaw and the point on the lower face.

Chin reduction is a cosmetic surgical procedure that reduces the size of the chin by shaving down the bone through an incision made in the lower gums. Chin reduction surgery begins with the surgeon making an incision inside the mouth (near the lower lip) or under the chin. Using specialized instruments, the surgeon detaches the bone at the tip of the chin and then reshapes and removes the excess bone to properly proportion the contour. The tip is then repositioned with the remaining section of chin and held together using wires and plates. The incisions are sutured to complete the procedure.

Malar Bone Reduction/ Cheek Bone Contouring


Cheek Bone Reduction is not a common procedure in transgender individuals among Caucasians, Hispanics or African Americans whose wide cheek structure is regarded as very attractive while in Asian individuals this is a very popular procedure since an oval-shaped face is considered to be more aesthetically pleasing and desirable.


Reduction Malarplasty is performed on patients with large cheek bones by either of two techniques. The first involves using a special drill to burr down or shave the excess bone at the zygomatic arch, the bony ridge that extends from beneath the eye socket along side the skull lateral to the temporal fossa. The second involves cutting out an appropriate amount of the zygomatic anterior arch, rotating or turning the lateral arch inward and securing with a small plate and two tiny screws. Both approaches maybe use depending on the problem.

Forehead Contouring


You can easily tell the difference between male foreheads from female ones. Covering a large portion of the face, the forehead shows a markedly different contour and projection in men than in females, although most people don’t realize how their forehead is shaped or even protrudes from the face.  

A female forehead shows a smooth and even surface while male forehead exhibit a pronounced brow bossing with greater hooding over their eyes and projects farther forward in front of the eyes.

Since the forehead contour is a distinct sign between both genders, forehead contouring is an important step in facial feminization surgery, thereby, creating a more naturally feminine appearance.

Scalp Reduction/ Scalp advancement


The foreheads and scalps are facial features that show more clearly whether a person is a male or a female. Male foreheads are remarkably different from female ones. Naturally, the distance from the hairline down to the eyebrow at the mid pupil line in males averages 2.6 inches in contrast to about 2 inches in women.

High hairlines and vertically long foreheads are generally accepted in males but not fashionable in females, therefore scalp reduction is performed as part of facial feminization surgery.

With scalp reduction, the portion of the scalp area is removed then pulled together, thereby shortening the forehead and minimizing the receding hairline. The best candidates for this procedure are individuals who suffer from male pattern balding and those who wanted to look more like a woman.

    VASER Liposuction

    Latest, third generation ULTRASONIC liposuction technique that is designed to virtually eliminate bleeding during surgery and to reduce inflammation and bruising. VASER Ultrasound technology literally liquefies the fat, which allows for greater precision and vastly superior results.
    Ideal candidate for Vaser Liposuction
    All candidates for liposuction should have realistic expectations of the results that can be achieved through the procedure and should be in good physical and mental health. The best candidate is someone who is in excellent health, is within 25 pounds of his or her ideal body weight, and has deposits of fat that have not responded to diet or exercise.   
    At the consultation
    You should express your desires and expectations from the surgical procedures. The surgeon will evaluate your medical health and explain the surgical techniques available to you. Your question will be answered as thoroughly as possible.  Examinations of your trouble are will be performed. The surgeon will go over the risks and potential complications of the procedure, as well as, the pre-operative and post –operative process.


    Risk Factors
    All surgeries carry some risks. Listed below are the risks or potential complications:
    The risks of VASER liposuction are very similar to the liposuction risks associated with any other method. However, because LipoSelection minimizes damage to other body tissues, patients typically experience less pain and fewer side effects than with other forms of liposuction. Serious liposuction complications are quite rare. Fat clots or blood clots may form, and there is a chance these will migrate to the lungs and cause serious illness. The fluid loss that results from liposuction will be anticipated by a qualified doctor, as it can cause low blood pressure, friction burns, or fluid buildup. The LipoSelection probe’s ultrasound energy may injure the skin or deeper tissues; although in most cases the benefits of the ultrasound system make it safer than traditional liposuction. If a doctor inexperienced with the LipoSelection procedure applies too much fluid, the wetting solution may collect in the lungs. If there is too much lidocaine in the liposuction solution, lidocaine toxicity may develop.
     
    Before the surgery
    At this time, you will be given an opportunity to ask any further questions. Continuation of healthy diet and exercise is very important before the surgery. Smoking must be stopped at least two weeks before the surgery
     
    On the day of the operation
    No eating or drinking for 8 hours before the surgery.  You should wear loose fitting clothing and sneakers. Do not wear sandals or slippers. You will meet with our nurse to prepare you for surgery. The surgeon will discuss the procedure with you again and do pre-operative planning. You will meet with our anesthesia provider to discuss anesthesia. You will be given a choice of day surgery or general anesthesia which required at least one day hospital stay depend on your choice but normally if it is an extensive surgery,   we required 4 days and 3 night’s hospital stay.
     
    Anesthesia used
    This procedure can be performed using local or general anesthesia.
     
     During the surgery
    First, the area to be sculpted is filled with a special saline solution that helps numb the site and shrink the blood vessels (to minimize blood loss and reduce bruising). This solution wets and fills the area to be treated, making fat easier to break up with the VASER System’s sound energy. Then a small probe transmits sound energy (similar to that used for cataract removal in the eye) that breaks up fat on contact, while leaving other important tissues relatively intact. Finally, gentle suction of the sculpted site removes the emulsified fat quickly. Thanks to its patented technology, only the LipoSelection procedure first emulsifies and then removes fat in this manner.
     
    Post Operative Expectation
    The treated area will be wrap with compression garments snugly and continuously for 6 days-2wks. Usually drains are placed removing excess fluid over a period of 2-3 days.
     
     Back to normal work
    Regular exercise work out may start after 2 weeks, start with light exercise then gradually move your way up to your regular routine as tolerated by your body.
     
    Pre-Operative instruction
    1. Consultation with surgeon
    2. Evaluation for other medical conditions
    3. ECG and Blood test will be taken
    4. No intake of food or water 6 hrs prior to surgery
    5. Compression garments should be available to be used post-op
    6. Stop smoking and alcohol intake at least 2 weeks before surgery
    7. Anticoagulants, vitamins, hormones (including birth control pills), anti-depressants and some other medications also to be stopped at least 2 weeks before surgery.
       
    Post-Operative instruction
    1. Wrap treated area with compression garments snugly and continuously for 6 days-2 weeks.
    2. A day after the surgery, patient may have light to moderate physical activities
    3. Some incisions will be left open and drainage is expected, absorbent pads should be used and changed frequently as needed.  
    4. Incisions will close by themselves within 2-3 days.  Use band aid to protect incision sites.
    5. Keep incision clean
    6. May have quick bath after 2 days post op. Regular shower when incisions are closed.
    7. May use regular girdle 2 weeks after compression garments.
    8. Do not apply ice packs or a heating pad to skin overlying the areas treated by liposuction
    9. Do not soak in a bath, Jacuzzi, pool or beach at last 7 days post-op
    10. Bruising may diminish after 2-3 weeks
    11. Return for check-up after 4 days removal of stitches.
    12. Swelling is expected, Maximal in the first 2 weeks, then gradually subsides.  Final result in 3-4 months.
    13. Skin may feel numb on the surface but insides are tender to touch.
    14. Some areas may become hard and lumpy; this is normal and will go away when fully recovered

    Metoidioplasty, sometimes referred to as a meto or meta or spelled "metaoidioplasty", is an alternative to phalloplasty for trans men. With the effects of testosterone treatment, the clitoris enlarges, over time, to an average of 4-5 cm (due to the clitoris and penis being developmentally homologous organs).
    In a metoidioplasty the enlarged clitoral tissue is released from its position and moved forward to more closely approximate the position of a penis.
    Metoidioplasty is technically simpler than a phalloplasty, and has fewer complications. The surgery itself is also considerably shorter (2-3 hours vs. 8-10 hours overall) and it is much less expensive. Unlike a phalloplasty, an erectile prosthesis is not needed to achieve erection, although the erection is usually less hard compared to the penis of a cisgender man. Because metoidioplasty can not achieve penile dimensions comparable to a cisgender penis, the neopenis is usually not capable of penetrative sexual intercourse. Genital orgasm is almost always retained after surgery (unlike phalloplasty which carries significant risk of loss of erogenous sensation).

    Liposuction


    Liposuction, also known as lipoplasty, liposculpture suction lipectomy or simply lipo ("suction-assisted fat removal") is the most common cosmetic surgery operation that removes pockets of fats from many different sites on the human body. It can remove stubborn areas of fat that do not respond to traditional weight loss method but remember it is not a substitute for dieting and exercise. It also involves the use of a stainless steel suction tube called a cannula. The cannula is pushed and pulled through the fat layer, breaking up the fat cells and suction them out by a powerful suction pump, thus improving body contour and enhancing self image. Areas of the body where liposuction is performed are: abdomen, hips, outer and inner thighs, flanks, love handles, back, knees, arms, submental (chin), Gynecomastia.
    The ideal candidates for liposuction are those who are physically fit, who exercise regularly and are not more than 20 pounds overweight. They may have bulging and flabby areas of the body from fat deposits, especially the abdomen, arms, thighs, and neck. Individuals with certain medical conditions may not be suitable candidates for liposuction.
    In all types of liposuction methods, there are certain things that should be considered when having the surgery. It maybe performed as an outpatient procedure or the procedure that will be done in a hospital and may require an overnight stay.
    The procedure is performed using a suction devise attached to a small, stainless steel tube called cannula. Through a small incision, the small bore cannula is inserted into fat layer between skin and muscle by alternately pushing and pulling it through the fat layer then suction it out by the use of a vacuum pump or a large syringe. The duration of the surgery will vary according to the amount of fat to be removed.

    Duration of the operation:
    The duration of liposuction Thailand depends on the number of areas, the size of the area and the amount of fat to be removed.

    Post operation care:
    1. A compression garment must be worn for two to four weeks.
    2. If patient had liposuction by local anesthesia, he/she may resume usual diet immediately.
    3. Drink adequate amount of water at least 8- 10 glasses a day or fruit juices to prevent dehydration.
    4. Bed rest is recommended for the first few hours after surgery.
    5. If non-absorbable sutures are placed, the surgeon will remove sutures after 5 to 10 days.
    6. Pain killers will be prescribed to control pain that may last as long as 2 weeks depending on the extent of procedure.
    7. Keep incision clean. Take a shower once or twice daily. Wound dressings need to be changed often. After one to three days, small self-adhesive bandages are sufficient.
    8. Do not apply ice-packs or a heating pad to skin overlying the areas treated by liposuction.
    9. Do not apply hydrogen peroxide or plastic Band-Aids to incision sites. Liposuction incision sites heal faster and better without topical antibiotics.
    10. Do not soak in a bath, Jacuzzi, swimming pool, or the ocean for at least 7 days after surgery in order to minimize the risk of infection.
    11. Patient taking Aspirin and Ibuprofen for at least 3 days after surgery. NSAIDs promote increase the risk of bleeding by impairing platelet function and depress immune response to infection.
    12. Drainage is placed for about 3-7 days depending upon the extent of surgery.
    13. Patient must maintain a proper diet and exercise regimen because the remaining fat cells could still enlarge and form cellulite thus creating irregularities.

    Recovery period:
    Depending on the extent of liposuction, patients are generally able to return to work between 2 days-2 weeks. Bruising will fade after 1-2 weeks. Swelling will occur and increase 5-10 days after surgery and should subside gradually over 2 weeks- 2 months while numbness persists for several weeks.

    Results:

    Possible risk and complications:
    With any type of liposuction procedure, complications may arise beyond the minor side effects but they are rare. Some possible complications include infection, allergic reaction to medication or material used during surgery, excessive fluid loss, thromboembolism, injury to the abdominal organs, and excessive loss of body heat (hypothermia), excessive intravenous fluids, aspiration pneumonia and cardiac arrest.
    It is the surgeon’s responsibility to give the written list of signs and symptoms to watch for, along with instructions for post-op self-care.
    Anesthesia use:
    The procedure uses local or general anesthesia
    Hospital admission:
    This procedure may be performed in the cosmetic surgeon’s office-based facility, an outpatient surgery center, or at a hospital. This procedure is usually done on an outpatient basis
    Preparation before the surgery:
    1. Patient and the surgeon will agree ahead of time on exactly which area(s) will be treated and both will discuss what outcome to expect. Assessment of the medical history, physical examination and laboratory tests will be performed during the consultation as well as the explanation of the course of surgery, recovery period, risk and complications.
    2. Eat a well balance diet several weeks prior to surgery. Over eating and crash dieting can greatly affect overall health status and well being of the patient.
    3. Daily exercises are an important factor to help enhance posture and strength at least 3-4 weeks prior to surgery.
    4. Smoking must be avoided for about 3-4 weeks prior to surgery, as nicotine interferes with circulation and will greatly affect healing process. Patient must be free of nicotine patch or nicotine based products for a minimum of 4 weeks prior to surgery.
    5.  Patient should avoid taking any medications such as hormones, anticoagulants, anabolic steroids and supplements at least 4-6 weeks to prevent complicating medical factors prior to surgery.

    6. Maintain the best of health and hygiene in the weeks prior to surgery. Colds, virus, throat infection or other illness can result to rescheduling of surgery.
    7. If general anesthesia or sedation will be used, and the surgery will be in the morning, fasting from midnight the night before is required. If only local anesthesia will be used, fasting is not required.

    Side effects:
    ·        Bruising: Can be painful in the short term, and should fade after a few weeks.
    ·        Swelling: is due to an inflammatory reaction to surgical trauma. This will occur and increase 5 to 10 days after surgery and should subside gradually over a month or two.
    ·        Scars: will vary in size depending on the particular procedure, and should fade over the weeks. Scarring is an individual thing, partly dependent on heredity. For some, scar healing may take as long as a year.
    ·        Pain: should be temporary and controlled by either over-the-counter medication, or by a prescription.
    ·        Numbness: sometimes persists for a few weeks.
    ·        Limited mobility: will depend on the exact procedure.
    ·        Nausea and vomiting: are among the side effects that may be associated with liposuction. Nausea can be caused by antibiotics, lorazepam, or local anesthesia.
    ·        Slight temperature elevation during the first 48 hours after surgery is a natural consequence of the body's reaction to surgical trauma.
    ·        Itching: may occur as part of a normal healing process. To help relieve itching, patient may try taking Benadryl as directed by the doctor

    • Assessment of the medical history (any allergies, serious medical condition and all medications taken both prescribed and non-prescribed), physical examination, and laboratory tests will be performed during consultation.
    • Blood and urine samples will be collected for routine preoperative laboratory tests. 
    • Smoking must be avoided for about 3-4 weeks prior to surgery, as nicotine interferes with circulation and will greatly affect healing process.
    • You will likely to be asked to stop drinking alcohol, a week before the surgery and throughout your recovery period.
    • Avoid taking any medications such as hormones, anticoagulants, anabolic steroids and supplements at least 4-6 weeks to prevent complicating medical factors prior to surgery. Avoid taking aspirin, anti-inflammatory drugs and herbal supplements as they can increase bleeding.
